How to Send Us Your Handpieces (If you don't already have our box)
Below are two items you will need to print out to get your handpiece to us. You may click on each item to open them off the site as a PDF, or you can "Right Click" and “Save As" to save them to your computer for printing.
Step 1:
Print & Fill out the Work Order Form - Place Inside Box
You may click on each item to open on your screen, or you can "Right Click" and “Save As" in order to save to your computer for printing.

Step 2:
Place handpieces in supplied bubble pouch. Tape box shut and put in the mail

Please print this out to give us an idea of what is wrong; what you would like done; and if you prefer a free estimate beforehand. We recommend keeping a copy for your records.

IF you Don't have a box with a label on it
Print the Business reply Label - Fill out the return address and affix to a box capable of protecting your handpieces on the way.

To request a box and/or a Pre-Paid USPS Shipping Label from Us - call 717-747-1319 or email us below. (We will send your Label via Email,Fax, or snail mail)
